Design: Open randomized trial. Methods: 60 patients with dermatochalasis were randomized into 2 arms 30 patients underwent upper lid radiosurgical blepharoplasty of both eyes and 30 patients for upper lid CO2 laser blepharoplasty of both eyes. Study period was from 1998 to 2002. In both arms, patients were pre-operatively admitted then underwent blepharoplasty under IV sedation. In radiosurgery arm, the procedures were done using an Ellman sugitron radiofrequency. In the CO2 laser arm, the procedures were done using a Coherent ultrapulse CO2 laser. Both instruments were used for skin incision, orbicularis resection, lipocontouring and fat resection. Outcome: Unit cost, direct cost, length of operative time, healing time, aesthetic expressed as bruising as a percentage of total wound. Results: In radiosurgery arm, mean operative time of 48.53 minutes, healed in 7 days with very minimal bruising. In CO2 laser arm, mean operative time off 50.63 minutes, healed in 12.5 days with minimal bruising. The costs are significantly lower in the RF group. Conclusion: Upper lid radiosurgical blepharoplasty is more cost-effective than upper lid CO2 laser blepharoplasty in terms of healing time and degree of bruising.
