Each heart case is a law unto itself, and as in many other diseases, one must proceed with care and deliberation, being careful not to overdo, and thus without intent, harm the patient. The dose should not be too small, but on the other hand, one must carefully watch for the cumulative effect of the agent, if it be too long continued, or the dose not properly regulated. A coupling of the radial pulse beat is a danger signal that must not be disregarded, for to do so may precipitate a serious cardiac emergency. Comment.-Dr. Nellans' caution as regards dosage is very timely. We may add, however, that we have never heard of a death resulting from the use of Specific Medicine Digitalis, nearly the toxic constituents of the drug being lacking in that preparation. If the latter, how many drops, and how often should it be given? Reply.-In our opinion Dioscorea is best administered in liquid form. Its absorption is more rapid, and its resultant action is therefore more positive and rapid. The dose of the Specific Medicine Dioscorea, which is the preparation we employ, is from two to ten drops in hot water, every fifteen to thirty minutes, to effect, depending upon the condition of the patient and the severity of the symptoms present. Associated remedies are Nux vomica, Colocynth, Lobelia and occasionally Gelsemium. Combinations of the various agents may be made as the symptons warrant. Echinacea is indicated in all depraved conditions of the system, and may be given in from two to ten drop doses, every three or four hours, depending upon the amount of sepsis present. THUJA FOR SWELLING OF FINGER TIPS AND WARTS Contributed ; Many young girls who play the piano come to me for a swelling of the finger tips. The symptoms are alike, marked by swelling of the fingers, which become red and inflamed. In most cases a wart appears on the middle finger, from which the inflammation seems to radiate. Touching the keyboard with the tips of the fingers produces intense pain, with tingling sensations. While the malady may seem trifling, it causes much hardship with those who play the piano for a livelihood. I recommend painting the entire inflamed surface with warm Specific Medicine Thuja twice a day. The wart is painted with Thuja four times a day, and every other day with strong acetic acid. As with any source of wealth, the allocation of remittances depends on incentives. When education costs are relatively low and schooling enhances labor market prospects, at home or abroad, the net rate of return to human capital formation is high. If the net rate of return to human capital formation is high low ; , additional remittances are likely to be allocated on the margin to schooling investments consumption ; . We also consider the effect of remittances on the unemployment rate of the labor exporting country. Remittances have two opposing effects on the labour market. First they augment the pool of funds for recipient households back home. This causes schooling to rise. Since it is likely that many households in labor exporting countries are credit constrained, remittances available for schooling investment will then relax these constraints and increase the level of the human capital stock. The second effect is on the return to human capital investment in the country receiving remittances. The effect of the rise in the human capital supply of households receiving remittances is to induce job creation, and to reduce the unemployment rate as non recipient households also increase schooling. As a result of the higher relative return to education, one effect is the substitution away from consumption and leisure towards human capital investment. At the same time, higher expected income has an opposite effect increasing the desirability of consumption and leisure, as they are normal goods. Mice were exposed for 28 days and immune parameters evaluated. Treatment resulted in an increase in absolute spleen weight at the high dose but no effect on thymus weight. No enhancement of the HI response was observed in the plaque assay or serum titers. Evaluation of HI, following 4 days of exposure to CEP, also failed to result in enhanced activity; however, a trend toward an enhanced response was observed in a CEP glycine extract preparation. No effects on NK cell activity were observed following 28 days of exposure. CMI, evaluated as MLR and antiCD3 proliferation, was not enhanced nor were splenic populations. In contrast, an enhanced CTL response was observed with CEP. Additional studies with the NTP water alcohol extract of EC also produced an enhanced CTL response. The purported immunostimulatory responses for EC and its recommended use for protection from flu or the common cold may be due to its ability to enhance CTL activity. As in other cells, after viral stimulation, IRF-7 is phosphorylated, forms homodimers, and is translocated from the cytoplasm to the nucleus where it acts as a transcription factor for IFN- genes 26, 29, 39 ; . Because IRF-7 plays a large part in the production of IFN- , it may also play a pivotal role in the regulation of this cytokine. To explore how cross-linking may impact IRF-7, we used intracellular flow cytometry and fluorescent microscopy to measure the levels of cytoplasmic IRF-7 and IRF-7 nuclear translocation, respectively. The results show that cross-linking CD4, BDCA-2, BDCA-4, or CD123 does not affect constitutive levels of IRF-7 and furthermore, does not inhibit HSV and IFN induced upregulation of IRF-7. However, using fluorescent microscopy, results suggest that CD4 cross-linking inhibits HSV-induced IRF-7 nuclear translocation. Although we have been successful in demonstrating nuclear translocation using fluorescent microscopy, this technique is not optimal. Nuclear translocation as determined by eye is very subjective and cannot be thoroughly quantitated. In addition, only a limited number of cells can be viewed using this method. To more comprehensively investigate the effect of cross-linking on IRF-7 nuclear translocation, we used the ImageStream imaging flow cytometer. This machine is a flow cytometer that captures up to six multispectral digital images of each cell. This technology allowed us to collect several hundred PDC images and objectively score IRF-7 translocation using image similarity analysis. Using the ImageStream, we show that cross-linking BDCA-2, BDCA-4, and CD4 inhibits IRF-7 nuclear translocation in a statistically significant manner, while cross-linking CD123 does not. It is not surprising that CD123 cross-linking behaved differently than crosslinking other receptors, as this was shown to induce PDC maturation, while others did not. This is further evidence that CD123 cross-linking works by a different mechanism to inhibit IFN- production. Because PDC that had CD123 cross-linked were still able to translocate IRF-7, IFN- may be regulated posttranscriptionally, possibly at the level of mRNA stability. Alternatively, the ability of IRF-7 to bind to DNA after nuclear translocation and act as a transcription factor may be affected. It is known that acetylation of the DNA-binding domain of IRF-7 inhibits its ability to bind DNA 47 ; . It also possible that while cross-linking CD123 does not affect IRF-7 in PDC, it might have an impact on another IRF important for IFN- production. Although IRF-7 is indeed an important player in the regulation of IFN- production, it is not the only one. IRF-3, and more recently, IRF-5, have also been shown to play a part in this pathway. Both are constitutively expressed by PDC and it is believed that different IRFs are induced by different viruses and lead to the production of distinct subsets of IFN- genes 38, 48 ; . Investigating this cross-linking phenomenon leads to the question, what is the in vivo correlate of cross-linking with Ab-conjugated microbeads? The answer may lie in the role that BDCA-2, BDCA-4, and CD4 play in PDC function. Although it has recently come to light that BDCA-2 acts as an endocytic receptor on PDC, its ligands have yet to be determined 19 ; . Given its endocytic nature, it would seem unlikely that the natural ligands for this receptor would solely serve to turn off the PDC. It is possible that the BDCA-2 Ab binds to a different epitope on the receptor than the natural ligand, leading to different outcomes. BDCA-4 has been recently identified as NP-1, a neuronal receptor for the axon.
Who developed croup symptoms shortly after taking a dose of echinacea may have been experiencing an allergic reaction. However, this result could also have easily been a chance occurrence; croup is not an uncommon disease in young children with URI symptoms.34 Further, placebo-controlled studies in adults have not documented any increase in rash or allergic reactions among echinacea recipients.20, 24, 25, 27, The statistically significant reduction in the number of subsequent URIs in children receiving echinacea compared with placebo recipients may be a spurious finding, given the multitude of outcomes examined. We had not planned on conducting this analysis prior to the study. However, the result is intriguing. It is conceivable that echinacea stimulated an immune response in study children that was too late to modify the URI for which it was given but provided a window of protection against another URI in the child. In one study, an echinacea preparation similar to that used in our trial was effective in preventing URIs in patients 13 to 84 years old, when given over an 8-week course.35 Given its lack of documented efficacy and an increased risk for the development of rash, our results do not support the use of echinacea for treatment of URIs in children 2 to 11 years old. Further studies using different echinacea formulations, doses, and dosing frequencies are needed to delineate any possible role for this herb in treating colds in young patients. Our finding that echinacea may be effective in preventing URIs also deserves additional study!
