Sir Cyril then turned to the question of Iraq and the implications of the current situation there for the UN. The territory of modern Iraq had been captured by the British from Turkey during World War I, and had been set up as a separate State, split into 3 provinces Mosul, Baghdad and Basra ; , and in 1920 a Provisional Government had been set up. But Iraq had never been a stable place, and only one of its Prime Ministers had died a natural death! Its King Faisal was assassinated in 1958, and since then the country had been ruled by the Ba'ath Party, led for the past 30 years by President Saddam Hussein. The Iraqi Government was highly unpopular in the country and in the region, but the country was rich in oil and strategically important. The world had closed its eyes and ears to the abuses of human rights and the belligerent noises coming out of Baghdad until Iraq invaded the neighbouring country of Kuwait. A coalition of 30 countries was created under UN authority to repel the invasion and restore the territorial integrity of Kuwait. The coalition succeeded in driving the Iraqi army back across the Kuwait frontier. It was correct in not pushing forward to capture Baghdad, since its mandate did not include the occupation of Iraq. But it was wrong not to honour the promises the USA had made to provide support to the Shias of southern Iraq in their efforts to topple the Government of Saddam Hussein. Following the Gulf War, the USA and the UK, and the French for a certain period, created no-fly zones in northern and southern Iraq, supposedly to protect the Kurdish and Shia populations in those areas. The zones had been enforced by air patrols and bombing action for more than 10 years, and had resulted in a considerable number of casualties on the ground. Cultures containing both spores and vegetative cells 18 hour and 30 day cultures were mixed. 24 hour cultures were also used to insure either a small number of spores or a lack of spores. In order to determine whether enough acid fuchsin was being carried over to the plates to inhibit growth, sufficient saturated acid fuchsin was added to agar to make a 1: 100 saturated solution. This is a more concentrated solution than was present in any of our plates. Bacterium coli, Bacterium cloacce, Staphylococcus albus, Sarcina lutea, Bacillus brevis, and Bacillus subtilis grew as well on this agar as in agar without the dye, a fact which proves that the action of acid fuchsin in our experiments was bactericidal rather than bacteriostatic. Molds grow readily on the surface of saturated aqueous acid fuchsin. It has been suggested that the reverse selective action of acid fuchsin may be due to the presence of the SO groups, 3 acid fuchsin 3 being originally produced by the process of sulfonation from basic fuchsin, which has the same selective action as gentian violet. Churchman states that "addition of the sulfonic acid groups not only makes the basic triphenylmethanes more toxic for Gram-negatives but less toxic for Gram-positive spore-bearing aerobes." But if the reverse selective action of acid fuchsin is purely a question of spore resistance, as our experiments indicate, one would expect to find that the process of sulfonation makes the basic fuchsin less toxic for the Gram-negative bacteria as well as for the Gram-positive organisms. PHA type II Gordon's syndrome ; is characterized by familial hypertension with hyperkalemia, slight hyperchloremic metabolic acidosis, and otherwise normal renal function. Thiazide diuretics are highly effective in this syndrome, commensurate with salt sensitivity. A multilocus linkage analysis11 yielded a lod score of 8.1 for linkage to chromosomes 1q and 17p. Interestingly, the chromosome 17p locus overlaps a syntenic interval in the rat that contains a blood pressure quantitative trait locus. PHA type II provides promise in leading to cloning of two additional as yet not appreciated genes leading to hypertension. Disse-Nicodeme et al.12 described a new locus on chromosome 12p13. They analyzed a large French family, in which 12 affected members over three generations confirmed the autosomal dominant inheritance. Affected subjects had hypertension together with long-term hyperkalemia, hyperchloremia, normal plasma creatinine, and low renin levels. Using powder gives a better concentration of antifungal agent miconazole or clotrimazole ; and the concentrations of the mupirocin and betamethasone remain higher. The combination is applied sparingly after each feeding except the feeding when the mother uses gentian violet ; . Do not wash or wipe it off, even if the pharmacist asks you to. In Canada, Kenacomb easier to find ; or Viaderm KC less expensive ; ointments not cream ; can be substituted for the above combination, but are distinctly inferior. I used to use nystatin ointment or miconazole cream 15 grams ; as part of the mixture, and these work well, but I believe the use of powdered miconazole or clotrimazole powder ; gives better results. 2. Gentian violet for treating Candida is discussed in handout #6. 3. Grapefruit seed extract Grapefruit seed extract can also be used for treating Candida as well. It can be used directly on the nipples and or orally. If used directly on the nipples, it should be diluted 5 to 15 drops in 30 ml ounce of water ; , applied on the nipples with a Q-tip or cotton ball, allowed to dry, and then covered, sparingly, with the all purpose nipple ointment. By mouth, grapefruit seed extract can be taken as a pill, 250 mg three times a day. 4. Treatments for Raynaud's phenomenon blanching of the nipple ; Raynaud's phenomenon is due to spasm of blood vessels preventing blood from getting to a particular area of the body. It occurs in response to a drop in temperature. Most commonly, Raynaud's phenomenon will occur in the fingers, typically when someone goes outside from a warm house on a cool day. The fingers will turn white and the lack of blood getting to the tips of the fingers will cause pain. Raynaud's phenomenon occurs more commonly in women than men, and is often associated with illnesses such as rheumatoid arthritis. Raynaud's phenomenon can also occur in nipples. In fact, it is much more common than generally believed. It can occur along with any cause of sore nipples, but it may also, on occasion, occur without any other kind of nipple pain at all. Typically, Raynaud's phenomenon occurs after the feeding is over, once the baby is already off.

In 100 per cent of the cases, smears from exudate presented typical pictures of the bacterial flora of Vincent's infection or trench mouth; namely, fusiform bacilli, comma or vibrio forms, filaments, spirilla, and cocci. In all cases organisms of this morphology were obtained in the first cultures from exudate. Isolation in pure culture of the growth that yielded fusiform bacilli was difficult and not successful in every instance. Spirilla were never isolated in pure culture. From one case presenting an extremely gangrenous condition of the gingivae, fusiform bacilli were isolated from the patient's blood. The cultural studies of this strain were reported in the preceding paper.'3 In thick areas of the stained preparations of exudate, a dichotomous branching type of growth was constantly demonstrated. This growth is shown in figs. 1, 2, 3 and 4. Continued branching seemed to occur at nodes or at thickened areas of a single branch fig. 3 ; . It may be so extensive as to form an agglomeration in which no detail can be discerned. Each branch appears to have a central shaft of mycelial filaments containing granules fig. 4 ; . In figs. 1, 2, 3, and 4 bacillary forms radiate from the central shaft. By Gram's method the filaments and granules may lose or retain the stain. They are vividly stained by heated carbol-fuchsin, and are usually pink by Giemsa's method. The bacillary forms vary in their morphology. Vibrio or comma forms, and fusiforms with pointed ends or having typical cigar-shape-square at one end and round or pointed at the other are in close proximity, adhering to the same central shaft. Masses of cocci adhere to the growth fig. 3 ; . In the large fusiforms and filaments, granules or gonidia-like bodies can usually be observed. Neither the body of the bacillus nor the granules retain Gram's stain. By Giemsa's method the body is blue, the granules pink or purplish. Associated with the bacillary forms are spirilla and cocci. The spirilla frequently adhere in thick aggregations to the sides of a branch. They lose the stain by Gram's method, and are best demonstrated by gentian violet or heated carbol-fuchsin stains. The cocci are amphoteric in staining reactions. The bacillary forms, filaments, and cocci, all vary widely in size.
