TA-03. FUSARIUM PNEUMONIA IN A NEUTROPENIC PATIENT AFTER CPT-11 FOR GLIOBLASTOMA Michael A. Badruddoja, Terry S. Peery, Jim Vredenburgh, David R. Reardon, Jennifer A. Quinn, Jeremy N. Rich, Sri Gururangan, and Henry S. Friedman; Duke University Medical Center, Durham, NC, USA Introduction: Fusarium pneumonia has typically been thought of as an uncommon infectious entity that is typically seen in the immunocompromised patient such as the neutropenic cancer patient acute leukemia or bone marrow transplant patient ; . In the past, these patients have accounted for 70% to 80% of the cases of disseminated Fusarium infections. To date, there are no reports of Fusarium pneumonia associated with CPT-11-induced neutropenia. We describe a patient being treated with CPT-11 for malignant glioma with grade IV neutropenia who developed Fusarium pneumonia. Case Report. A 55-year-old man initially presented with progressive weakness of his right upper extremity followed by focal motor seizure. An MRI of the brain obtained, which was significant for a left frontal mass. The patient underwent resection with Gliadel placement. The patient's clinical course was complicated by the development of ventricular tachycardia, which required the use of an automatic implanted cardiac defibrillator AICD ; . The patient completed radiation concurrent with Temodar and subsequently received two cycles of lomustine and two cycles of Temodar. The patient received two infusions of CPT-11 and soon after began experiencing pre-syncopal syncopal episodes with frequent AICD firing. It was thought that this was the result of side-effects related to CPT-11 causing symptomatic bradycardia, coupled with decreased intravascular volume related to CPT-11-induced diarrhea. The patient received fluid but continued to have pre-syncopal syncopal events. The patient's total white blood cell count dropped precipitously from 11.4 to 1.4, with a nadir of 0.9 ANC 522 ; , accompanied by significant thrombocytopenia seven days after second infusion of CPT-11. As a result of persistent pre-syncopal syncopal events, the patient presented to the emergency department. The patient denied any shortness of breath, though he did complain of a persistent dry cough. The patient had a chest X ray performed during the ER evaluation, which revealed nodular infiltrate at the left lower lobe. The patient was admitted, and a chest CT confirmed nodular area at the left lower lobe. To more adequately evaluate the chest finding, a bronchoalveolar lavage was performed. A fungal culture of the specimen submitted revealed Fusarium. The patient was supported with G-CSF, broad spectrum antibiotics, as well as an antifungal agent voriconazole ; . The patient's course was further complicated by the development of wide complex ventricular tachycardia with prolonged QT-interval. As a result, voriconazole was discontinued and the patient was placed on liposomal amphotericin B. The patient was supported with growth factor, antibiotics, and was placed on Coumadin. The patient was discharged after a threeweek hospital stay on liposomal amphotericin B. Conclusion. Fusarium pneumonia may be a more common entity than previously recognized and may be seen in patients other than leukemia patients or bone marrow transplant patients. All neutropenic patients may be at risk for the development of less common mycotic infections. Fusarium is an angio-invasive mycotic organism that is associated with a high mortality rate, particularly in those patients that have persistent neutropenia related to chemotherapy and corticosteroids. Anti-fungal agents used to treat life-threatening mycotic infection have significant side-effect profiles and drug interactions in patients with complicated medical regimens and need to be used cautiously. patients in the 125-mg m2 non-EIAC and at the 225-mg m2 EIAC strata, respectively. Durham, NC Epidural infusion of anesthetic agents is one of the most common forms of analgesia available to laboring patients. There have been several studies published reporting that the average distance to the epidural space is less than 4.9 cm. However, we do not believe that this number accurately reflects our clinical experience. In addition, we hypothesize that there are differences in the distance to the epidural space amongst various ethnic groups and that the distance increases proportionally with increasing Body Mass Index BMI ; . To test these proposals, we conducted a retrospective chart review of all parturients who received an epidural catheter at our institution in 2001. We collected information on the recorded distance to the epidural space, demographic data height and weight ; , and patient ethnicity. Each patient's BMI was calculated using the standard equation weight kg ; height m2 . The data were evaluated using standard and descriptive statistical techniques. The effects of BMI, race, and their interactions were tested with multiple linear regression. Our principle ethnic groups were African American, Hispanic and Caucasian. Mean epidural distances for all three ethnic groups were significantly greater than 4.9 cm. Furthermore, epidural distances increased significantly with increasing BMI; this effect was strongest amongst African Americans in comparison to the other racial groups. To test the hypothesis that E3.10 and J3.8 are alleles of RhoA, we performed complementation analysis between each of the two point mutations, E3.10 and J3.8, and each of two RhoA excision alleles, 72F and 72O Strutt et al. 1997 ; .
