Although he was probably cured of his cancer, his postoperative course was slow and difficult because of his hemiplegia and lack of communication as a result of the laryngectomy. Six weeks after surgery, he was transferred to a rehabilitation care facility. These strokes occurred either in the intraoperative or the postoperative period. Pre-existing cerebrovascular disease and haemodynamic instability were responsible in the first and second cases, and cardiogenic embolization probably was responsible in the third. Strokes associated with general surgical procedures are uncommon and are rarely reported.1 The purpose of this paper is to review the incidence and mechanisms of perioperative stroke in various situations encountered in the surgical patients. General overview Stroke is defined as a focal neurological deficit which has a sudden onset and which persists for longer than 24 hr. Global cerebral ischaemic syndromes caused by hypoxaemia or ischaemia are not considered. The overall incidence of perioperative stroke is low. In 1986, Cohen et al. published the outcome of a nine-year postanaesthetic followup program, involving 112, 721 anaesthetics at the Health Sciences Centre in Winnipeg. Perioperative stroke was not listed as a separate complication.2 Although stroke in the perioperative period is uncommon, the acute mortality is high - from 16 to 50% 3 ' 4 and many of the survivors have long-term disability, resulting in pain and suffering to both the patients and their families.5"7 There are two mechanisms of stroke: 1 ; occlusion of a cerebral artery thrombotic or embolic ; , which causes brain infarction, and 2 ; spontaneous rupture of an intracranial artery, which causes bleeding either in the brain parenchyma or in the subarachnoid space. Based on the data from the National Pilot Stroke Data Bank 1982 ; and the Harvard Stroke Series 1978 ; , the ratio of infarcts to haemorrhage is about 4: 1, and embolism accounts for approximately one-third of all strokes.8 The major risk factors for stroke identified by the special subcommittee of the Council on Cerebrovascular Disease of the American Heart Association were transient ischaemic attacks, cerebral infarction, hypertension, car.

Appropriate experimental models of GI disease and clinical proof-of-concept studies are required to critically evaluate the quantitative contribution the sensory neuron-specific targets make to the induction and or maintenance of GI hyperalgesia and whether or not modulation of a single target is therapeutically sufficient. With these considerations in mind, the following sections of this article will discuss a select group of sensory neuron-specific receptors, sensors and ion channels that have potential in the therapy of visceral hypersensitivity and are currently the focus of interest.

Simpson 2 ; 1 ; United Institute of Physics of the Earth, RAS, 2 ; United States Geological Survey, mikh uipe-ras gis Comparing subsidence of the Great Valley with that of 150 m.-y.-old oceanic lithosphere model accounts growing sedimentary layer and crystallisation of basalt melt at the top of the asthenosphere ; , we found that subsidence of oceanic lithosphere is more rapid in comparison to main parts of the Valley, but fits well to the Sacramento delta region subsidence, heat flow and the maximum paleotemperature of sediments. We suggest that during J 2 - 3 when the Great Valley oceanic lithosphere was obducted onto the lithosphere of the Sierrian volcanic arc, the Sacramento block was swung clockwise with respect to the San Joaquin block, opening a triangular zone now occupied by the Sacramento delta depocenter. Being free of the obducted oceanic plate, its lithosphere has been thinner and hotter than in other parts of the Valley. We used non-steady state temperature to calculate strength profiles for the Sacramento delta area, where hypocentral depths reach 23 km as compared with 1215 km in the Coast Ranges. For a diabase rheology and relatively low strain rate, the upper part of the crust is brittle up to the 21 km depth. As a result the lithosphere of the delta depocenter is weaker than in other parts of the Great Valley. Strain can concentrate in this area, which is brittle to a depth of at least 20 km and pilocarpine and pergolide.
