POLICY It is the policy of the DuPage County Health Department to allow employees to take a Personal Leave of Absence without pay for extraordinary circumstances of personal need when it is determined to be in the best interest of both the Health Department and the requesting employee. A. Background: This manual section describes changes to coding and payment for drug administration for hospitals paid under the OPPS, to be implemented in the January 2006 OPPS update. The January 2006 OPPS OCE and OPPS PRICER reflect the changes identified in this manual section. The instruction to install the January 2006 OPPS PRICER is provided in Change Request 4250, Transmittal 786, dated December 16, 2005. The instruction to install the January 2006 OPPS OCE is provided in Change Request 4238, Transmittal 784, dated December 16, 2005. Unless otherwise noted, the coding and payment policy addressed in this manual revision are effective for services furnished on or after January 1, 2006. B. Policy: Certain drug administration services furnished under the Hospital Outpatient Prospective Payment System OPPS ; prior to January 1, 2005 were reported using HCPCS alphanumeric codes: Q0081, Infusion therapy other than chemotherapy, per visit; Q0083, Administration of chemotherapy by any route other than infusion, per visit; and Q0084, Administration of chemotherapy by infusion only, per visit ; in combination with applicable CPT codes for administration of non-infused, non-chemotherapy drugs. Note: HCPCS code Q0085, administration of anti-neoplastic drugs by both infusion and a route other than infusion, per visit, was discontinued in 2004. ; These same drug administration services furnished by hospital outpatient departments to Medicare beneficiaries during CY 2005 were reported using CPT codes 90780, 90781, and 96400-96459. Payments for these drug administration services in 2005 continued to be made on a per visit basis rather than a perservice basis ; due to the per-day 2003 cost data available to set CY 2005 payment rates. Effective January 1, 2006, some of the CPT codes that were used for drug administration services under the OPPS throughout CY 2005 are replaced with more detailed CPT codes incorporating specific procedural concepts, as defined and described by the CPT manual, such as "initial, " "concurrent, " and "sequential." In order to facilitate the transition to more specific CPT codes within the hospital environment and to assist hospitals in ensuring continued correct coding concepts, drug administration services provided in CY 2006 under the OPPS will be billed using a combination of CPT codes and C-codes that were created to be consistent with some aspects of the CY 2005 CPT coding structure. Hospitals are reminded to bill a separate Evaluation and Management code with modifier 25 ; only if a significant, separately identifiable E M service is performed in the same encounter with OPPS drug administration services.

WARNINGS: Because of its anticholinergic activity, amitriptyline hydrochloride, and therefore ETRAFON Tablets, should be used with caution in patients with glaucoma and those in whom urinary retention is present or anticipated. As with other drugs, ETRAFON Tablets should be used with caution in pregnant women. Dosage for children has not been determined; ETRAFON Tablets are not recommended for use in children at this time. Monoamine oxidose MAO ; inhibitors may patentiate certain effects of other drugs and, in so doing, cause death. In patients who have been receiving a monoamine oxidase inhibitor, it is recommended that two weeks or longer elapse before the start of treatment with ETRAFON Tablets, to permit recovery from the effects of the MAO inhibitor and to avoid possible potentiation. Treatment with ETRAFON Tablets should be initiated cautiously in such patients, with gradual increase in dosage until a satisfactory response is obtained.

Absolutely not. Unfortunately, this misinterpretation was widely disseminated through the media when the Phase 1 results were published in 2005. In fact, the study strongly reinforces what consumers and families have known for years--that there is no "one size fits all" treatment for schizophrenia. Moreover, the study demonstrates that significant further research is needed into better treatment options for individuals and the impact that factors that are not measured in randomized controlled-trials have in successful clinical and functional outcomes. In the CATIE trials, no one drug was found to be effective for the majority of consumers. This reinforces the position that "fail-first" policies are clinically unsound. While study participants across the medications experienced a range of side effects, there were differences between drugs in the types of side effects that caused treatment discontinuation. Clown for my terrible joke telling, so it seemed a natural thing to do to wear the costume. Hopefully this and the crutches ; will make me stand out on the telly too. The physios at the local hospital had some special extralong elbow crutches done for me as I'm 6'4" tall, and they'll change the rubbers on the ends of my crutches before the big day.
